Forms and ApplicationsFRIENDS has several forms and applications in PDF. Some must be printed and mailed back to the FRIENDS office at the address below. The Maintenance Activity Form for trail work can be completed within the PDF and submitted online. The Litter Control Hours Form can be completed within the PDF and submitted online. The Volunteer Agreement form below must be printed out and mailed to the FRIENDS office at the address below. The Volunteer Agreement Form is required by the National Park Service for volunteer work on the Parkway. Mail to: P.O.
